I haven't been to the doctors for over 30 years so I am not sure what the procedure is any more and I have multiple issues I need addressing that require me to bite the bullet and make an appointment.

 

I need the doctor to confirm in writing I have

1, colour blindness

2, fear of heights

3, motion sickness over long distances

 

I am not sure how apart from colour blindness he would be able to diagnose the other problems, will the doctor just deal with all 3 in a single session and accept my word , will he even entertain such a request, would there be a charge if I am in receipt of JSA ?

If you haven't been to the doctors in over 30 years, are you sure you are still registered with that practice?

You need to register with your local practice/doctors surgery but please bear in mind that as you will be a new patient they will have to request your medical records from the nhs central registrar which may take time so bear this in mind .

Yes, thank you, I have already found the surgery I wish to use but they are requiring I provide photo ID such as a passport or driving licence and I do not drive or travel abroad so I have neither.

 

Frustrating to say the least and as I am low income and cannot really afford to buy either.

You can get referred to the eye hospital via the opticians for the colourblindness test. Although, the opticians themselves might do them. (have only had them done at the eye hospital)

Thank you for that information, if the doctor is unable to confirm that I am indeed colour blind, I shall take the route you suggested.

If the GP's says "we don't have an Isihara test book" (or, we should have it but it is so rarely used we've mislaid it"!)

would it save a referral to suggest they test you using an online version?

Thanks, I will give that a shot should the doctor be unable to perform the test themselves, I know I took the test as a child so there might be a chance it is recorded on my file but not sure if that was the school nurse or my actual GP.
