She could add the correct and missing data to the minutes and send it back tot hem as a more accurate record. Don't sign though until they are happy their side also.

It's normal if they missed stuff out or the investigating officer's competence is in question...

 

but you said this was a disciplinary, not an investigation?

In reality it is both a disciplinary and investigation process ?

 

The company are looking to blame someone for what happened, that it is a failure of an employee, rather than an issue of the company having poor management systems or care processes in place.

 

The company will be wanting to file a report saying "Mrs J Bloggs failed in her supervision of care staff and the vulnerable people being cared for. That Mrs J Bloggs failed to comply with company standards................" 

 

It is likely the company will be wanting to satisfy any outside authority that is responsible for care standards, that it has taken all necessary actions. Pointing blame at a supervisor who has left the employment of the company must be a temptation here, rather than report on several issues, such as a faulty door, inadequate company procedures etc. 

 

Which is why cautious actions to try to avoid the company blaming one person,  must be continued. If there is another person who has visited the address, who was aware of the faulty door, it may be worth them providing a written statement, if they are willing to do so. This is provided the faulty door was reported to the care company and they failed to take necessary action to get it fixed.
