Best to go on twitter or other social media site and confirm the Halifax SAR address before sending the request. I lodged a SAR in October 2015 by email, then again in December 2015, by sending 10 GBP cheque to:

Data Subject Access Request Team

Drop Zone C41

Charlton Place

Charlton Road

Andover

Hants

SP10 1RE

They do not accept any correspondence by e-mail and claim not to have a fax number. They refused to accept a 10 GBP payment through Bank Transfer and insisted that I fill out a form when I asked for ALL my data. They then sent the SAR data to my previous address AFTER the 40 day deadline.

I lodged a complaint with the Information Commissioners Office (ICO). ICO found that my e-mail SAR was a valid request and that Halifax should have responded within the 40 day period. I asked the ICO to review the complaint for the other elements that were missed. e.g. asking to complete the form, data sent to previous address amongst other things.

ICO has warned Halifax about responding to SAR's within 40 days and refusing to accept a SAR through e-mail.

They also should not be asking anyone to complete a specific form. If the original request states clearly that it is a Subject Access Request, then it is wrong of them to stall by asking you to complete templated form.
