not sure of my next steps, do I send that form to citizens? Do I ask highlight the cardholder repayment protection and charges all together or separate? What do I state on letter? I have no experience of any of this? Is there any sample letters I could use to help me? Any advice welcome.

Received my DSAR from CitiFinancial re; platinum credit card taken out in 2003. received copies of statements and original application form, which includes a ticked box for "Cardholder Repayment Protection" which statements show I paid an average of £20 per month , is this the same as PPI?? I cannot remember asking for this. the statements also shows numerous Late Charges and Overlimit Charges for £20 a time. Also, £5 charges for cash advances. Many advice would be welcomed.
